OVERVIEW:

Root Cause Analysis (RCA) adalah sebuah proses yang dirancang untuk dimanfaatkan dalam melakukan investigasi dan pengkategorian berbagai akar masalah atas events yang memiliki dampak bagi keselamatan, kesehatan, lingkungan, kualitas, kehandalan dan produksi. Istilah kejadian digunakan secara umum untuk mengidentifikasi kejadian-kejadian yang menghasilkan atau memiliki potensi menghasilkan beragam konsekuensi. Ringkasnya, RCA adalah tools yang digunakan untuk mengidentifikasi tidak hanya what dan how sebuah event terjadi, tetapi juga why event tersebut terjadi. Hanya ketika investigator mampu menentukan why sebuah event terjadi atau sebuah kegagalan terjadi, mereka akan mampu menspesifikasi tindakan-tindakan korektif yang akuntabel sekaligus mampu mencegah terjadinya kembali kegagalan tersebut dimasa mendatang. Pemahaman atas why sebuah event terjadi menjadi kunci utama bagi pengembangan rekomendasi yang efektif.

COURSE DESCRIPTION

Pasal 39 dari UU Rumah Sakit No 44 Tahun 2009 mengamanatkan bahwa “dalam penyelenggaraan rumah sakit harus dilakukan audit, dapat berupa audit kinerja dan audit medis.” Lebih lanjut bagi RS yang menerapkan PPK-BLUD berdasarkan Permendagri No. 61 Tahun 2007, Bab XIV Pasal 123 menegaskan bahwa pengawasan operasional BLUD dilakukan oleh pengawas internal atau auditor internal yang berkedudukan langsung di bawah pemimpin BLUD.

Namun kenyataannya, kegiatan audit internal dalam lingkungan RS berjalan lamban dibandingkan kegiatan lainnya. Salah satu penyebabnya adalah belum ditemukannya konsep audit internal sehingga kesatuan pandang, sikap dan pelaksanaannya atas aplikasi audit internal RS masih kurang intensif dan beragam. Akibatnya pelaksanaan kegiatan audit internal di RS belum berjalan, atau sudah berjalan tetapi hanya seadanya saja, sehingga tidak mencapai tujuan dan fungsi pengendalian internal yang sebenarnya.

INTRODUCTION:

Protection of low, medium and high voltage power systems requires an understanding of system faults and their detection, as well as their safe disconnection from the power system. INTRODUCTION:

This training course presents reactive power compensation and an overview of modern practices in raising efficiency of power systems through application of reactive power and voltage regulation.

This training course starts with discussion about the structure of power systems from a power plant to the customer, and typical power system voltages in different parts of the system. Then, it explains how power losses and voltage drops in the power system may be reduced through compensation of reactive power either by shunt capacitors for an inductive load or shunt reactors for a capacitive load. Arrangement of a shunt capacitor bank and a sequence of its design is considered as well. Explanations are supported by numerous drawings, photos of actual equipment and the example showing how to design shunt capacitor bank.
